Observation

During annual inspection on site, provider was unable to present copy of completed food handlers permit. During annual inspection on site, licensor observed MERIT/workforce training with director. No BBP training since 2015 and for two staff and 2021 for provider. Violations observed with provider. Trainings completed on 11/01/2023.
